	Fort Myers Indoor Lacrosse Participation Form Please complete the following form and electronically "sign" it by submitting it to Fort Myers Indoor Lacrosse. This is a registration form and waiver of liability. You must send a check for payment prior to start of play. The address to which to send the check is: 12593 Shannondale Drive, Fort Myers, FL 33913. *Note: mail in Fort Myers and Naples can take up to three days to arrive...plan ahead. No pay, no play.* You will be required to sign a printout of your form on the first night of play. Minors must complete the form found here and bring it with their parent's signature.

I understand that lacrosse is a dangerous sport in which serious injury is always a possibility and hereby assume all risk associated with play. Further, I hereby grant full permission to any and all of the foregoing to use my name and any photographs, videotapes, motion pictures, recordings, and any other record of this event for any legitimate purpose, including but not limited to marketing and promotion of the league, products, and/or sponsors associated with the league. I understand that if I am under the age of 18 that I must have the permission of my parent or legal guardian to participate in Fort Myers Indoor Lacrosse League play. I, the undersigned parent or legal guardian, hereby consent to the applicant's participation and waive and release all rights and claims for damages as is more fully set forth above.
